Prosteren: for treating benign prostatic hypertrophy

Serenoa Repens

Serenoa repens, commonly known as saw palmetto, is the sole species currently classified in the genus Serenoa. It is a hearty plant; extremely slow growing, and long lived, with some plants possibly being as old as 500-700 years. The fruits of the saw palmetto are highly enriched with fatty acids and phytosterols, and extracts of the fruits have been the subject of intensive research for the treatment of benign prostatic hyperplasia (BPH). This extract is also commonly used for other medical conditions, such as baldness, polycystic ovarian syndrome, and other hyperandrogen conditions, though research on such uses is weak or absent.

Serenoa Repens has the following properties:
- Inhibition of 5 alpha reductase, the enzyme that converts testosterone into the more active dihydrotestosterone
- Inhibition of the binding of dihydrotestosterone to specific receptors
- Anti-estrogenic activity
- Reduction of prostate growth induced by prolactin
- Anti-inflammatory activity
- Anti-edema activity

With advancing age the hormonal changes that occur in humans contribute to determining the onset of benign prostatic hypertrophy (BPH). As testosterone decreases several other hormones increase (FSH, LH, estrogen and prolactin). In particular the enzyme 5 alpha reductase increases, which has the function of converting testosterone into dihydrotestosterone more active and accumulate in the prostate.

As part of drug treatment of BPH the lipid extract of Serenoa repens has held an important position, compared to synthetic molecules, and performs a series of actions that enable more effectively and synergistically to relieve the symptoms and the disorders related to BPH.

Serenoa repens has been available to the doctor for several years but its clinical effectiveness is constantly evaluated in clinical trials. Recently there has been a meta-analysis of 18 studies, including 16 double-blind studies, with a total series of 2,939 patients, which confirmed systematically the effectiveness, at least equal to comparison therapies, in improving symptoms of the urinary tract and of the urinary flow, while highlighting the superior tolerability of Serenoa repens in particular regarding sexual dysfunction.
